Showcasing results and future perspectives in type 2 diabetes care

The CARE4DIABETES Final Conference will bring together representatives from Ministries of Health, EU institutions, international organisations, healthcare providers and project partners to present the results of the CARE4DIABETES Joint Action and discuss how lifestyle-based interventions can transform type 2 diabetes care across Europe.

CARE4DIABETES is an EU-funded Joint Action aimed at reducing the burden of type 2 diabetes by transferring and implementing, in 12 EU Member States, an evidence-based and multidisciplinary lifestyle treatment programme. The initiative is based on the Dutch good practice “Reverse Diabetes2 Now”, which focuses on sustainable lifestyle changes to improve health outcomes and quality of life for people with type 2 diabetes.

• Findings on evaluation, sustainability and scalability at national and EU level
