In Greece, healthcare professionals and participants of the CARE4DIABETES (C4D) project are transforming the management of type 2 diabetes through a multidisciplinary, lifestyle-based approach. Their experience shows how active education, close support, and teamwork can make a real difference in people’s health and wellbeing.

The Greek C4D team — involving professionals from the 1st YPE Attica and Alexandra Hospital — has developed a model that blends medical expertise with motivation and emotional support. “At C4D we are not just teaching; we are empowering, inspiring, and walking with you on the journey to a healthier and happier life,” explains one of the professionals involved.

Participants highlight how the programme is helping them reshape their daily lives. “With C4D’s guidance and support, I am transforming my habits, reclaiming my health, and living stronger every day,” shares one participant.

Beyond clinical care, CARE4DIABETES promotes patient empowerment, strengthens autonomy, and builds meaningful connections between professionals and the people they support — proving that collaboration is key to achieving lasting change.
